Grant Opportunity: RHTP

Indiana is investing in the future of rural health. Through the Rural Health Transformation Program (RHTP), eligible physicians can receive up to $300,000 over five years for committing their full-time clinical practice to a designated rural community. This initiative strengthens access to care, supports rural providers, and helps close critical workforce gaps across the state.

This Rural Health Transformation Program is supported by the Centers for Medicare & Medicaid Services (CMS) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $206,927,896.80 with 100 percent funded by CMS/HHS.
